The enrollment form is the primary document providing us with information about you and your child. The form should be filled out completely. Please note that both the mother and the father must both sign the form and be listed as those who can pick up the child enrolled. Only if a parent does not have legal custody of the child will the center not allow a parent to pick up a child. In such a case, the form must be clearly marked by the name of the parent who does not have legal custody, "Does not have legal custody."
